| GP Referral Programme
The GP referrals are 12 week exercise programmes based at Hillsborough Leisure Centre for customers who have been referred by a GP or Health Professional.
But how does the GP referral programme work?
On receipt of a correctly completed referral form from the correct source the patient will be sent a welcome letter and overview of the scheme.
If you have been referred we will contact you with an offer of an appointment with a qualified instructor who will assess you. All our instructors are qualified to a minimum of YMCA Gym Instructor or equivalent, Level 3 Fitness Knowledge or equivalent, Exercise Referral Qualification (YMCA, Gayton, Wright Foundation) and BACR Phase 4 (for CHD Patients).
During the appointment we’ll ask you to complete a brief questionnaire to find out your feelings on being referred, what you expect to achieve from the scheme, what exercise you may have done in the past, what you enjoyed and what interests you now.After this you will receive a letter which you need to show at reception to gain access to the centre for 12 weeks. Dependant on the severity of the illness the instructor may opt to carry out further one to one sessions with you in the gym to closely monitor your progress.
Certain illnesses will fall under the remit of Inclusive Fitness Initiative qualified staff and patients with these may be referred to relevant gym staff. Consent will need to be obtained from the patient for the GP Referral Instructor to share relevant information.
If you choose to attend fitness classes you will be advised which will be suitable for and with your consent, the instructor will relay relevant information to the class coach concerned.
At week 6 you’ll be asked to complete an evaluation form asking how you feel you are progressing with the scheme. At the end of your 12 weeks you’ll be asked to complete another evaluation to see how the scheme has worked for you. This information will then be put together and fed back to the original referrer.
